Project Name: Hillhurst-Sunnyside Needs & Preference Study

Purpose of the Study

We would like to ask your assistance in determining the future direction of the Hillhurst-Sunnyside Community Association, by filling out the following anonymous survey.

The survey will take between 10 and 20 minutes to complete.

The final report should be available in the fall and will be put on the web site: hillhurstsunnysidecommunity.com and included in our Community Newsletter, the VOICE.

Privacy and Access to the information

We will be asking a few personal questions on this survey. Feel free to provide or not provide this information. We do not perceive any potential risks for you in participating in this survey and no information that may specifically identify any individual will be utilized. The benefit to you is that your input will be used by the Hillhurst-Sunnyside Community Association for Community development and planning.

Participation is completely voluntary. The Community Association will have access to the raw data and dispose of the questionnaires 3 months after the final report has been completed.

Prizes for participation

All returned surveys will be put into a draw for prizes to be awarded in September, 2008. Should you prefer to contribute your thoughts electronically through this online form, your email address would then be entered in the draw
